De-brief Meeting Scabies Outbreak November 2, 2012

Staff Present: M Baker, A Skinner, B Carter, K Knox, S Karp, M Lake, J Shillington, T Mercer 1.Stacey gave a brief summary and reported that the treatment was very successful. 2.Staff present were asked to share what was done well and what could have been improved (see attached). Recommendations:

  1. Ensure all communications are in writing and include action plan. Be sure to post in staff locker room where all staff swipe in.
  2. Develop checklist for each department.
  3. Always treat the residents twice.
  4. Limit amount of clothes in wardrobes and drawers. Look at process for families to change clothing according to seasons.
  5. 2 extra nursing staff sufficient for day after treatment applied.
  6. Ensure privacy curtains are changed
  7. NP to provide feedback to Public Health re. draft guideline.
  8. Revise Fairmount’s policy.

What worked well

  1. Extra staff provided
  2. Housekeeping stripping and cleaning beds
  3. 3 days’ supply of clothes removed from rooms prior to treatment and laundered to be ready in am.
  4. DRC present both days for consistent communication & direction
  5. Comforters, etc. not returned to rooms until after 2nd treatment
  6. Staff worked hard, no complaining and so kind to the residents

Scabies Reports

What could be improved

  1. Label upholstered chairs before removing
  2. Include footwear with 3 day supply of clothes and ensure clean footwear available for immediately after treatment is applied
  3. Setting up in shower room with all necessary supplies (Incontinent products, garbage, gowns, gloves, etc.
  4. Work in pairs to apply lotion (one needs to be regular staff who knows residents}
  5. Designate someone to make the beds after cleaned
  6. Day staff did not know about 3 day supply of clean clothes in laundry room
  7. Cart with clean clothes brought to service area outside unit but wasn’t covered.
  8. Carpets not cleaned right away
  9. Night of initial treatment, beds cleaned and many closets emptied but other closets not emptied and rest of furniture, etc. in rooms not cleaned until next day
  10. Lack of co-ordination to ensure that residents being treated and beds being cleaned matched up.
